According to Linn-Mar Community Schools policy and Iowa state law all students are required to have an immunization record or waiver form on file in our Health Office in order to attend school. In addition, 7th graders are required to have 2 immunizations given prior to the beginning of their 7th grade year. A Tdap and a Meningitis immunization are required by law before a student can begin their 7th grade year. We need to have on file in our Health Office record of their Tdap and Meningitis vaccination or a completed medical/religious waiver prior to the first day of school. If we do not have documentation of the updated Tdap and Meningitis immunization or the medical/religious waiver on file, your student WILL NOT be allowed to attend school until the immunization has been received.
Iowa law states:
All students entering, advancing or transferring into 7th grade and born on or after September 15, 2000, will need proof of an adolescent tetanus, diphtheria, and pertussis (whooping cough) booster immunization for school in the fall.
The requirement for the Meningitis vaccination has been in place since January 2017. The law change requires a one-time dose of meningococcal vaccine received on or after 10 years of age for students in grades 7 and above, if born after September 15, 2004. Also to make you aware a second dose will be required during high school for entry into the 12th grade.
Please take this into consideration when planning your students doctor visits prior to 7th grade. Ask if they have already received the immunization. If they have, please get a copy from your health care provider and bring it to us. If you would rather, you may have them fax it to our office at 319-373-3222.
